What is Form 1095?

Form 1095 is used to report information to the IRS about individuals who are covered by minimum essential coverage. Employers with less than 50 full-time employees will file a 1095-B, and employers with 50 or more full-time employees will file 1095-C.

What is Form 1094?

Think of Form 1094 as a “cover sheet” for Forms 1095. Form 1094 must be used to report to the IRS summary information for a group and to transmit Forms 1095 to the IRS. Employers with less than 50 full-time employees will file a 1094-B, and employers with 50 or more full-time employees will file 1094-C.

Who files Forms 1094 & 1095?

Our portal automatically creates a form 1095 for each employee on the plan, and a form 1094 for the entire group. However, the employer is responsible for filing these forms with the IRS. These forms are due electronically by March 31st.

How can I download Forms 1094 & 1095?

Employers should enter the Compliance tab on the portal to obtain all 1095 forms for their subscribers. The dropdown menu allows employers to view tax forms from previous years.

The employer may distribute these forms to their subscribers if needed, but members can also access their own 1095 in their member portal. Find information on member basics here.
