What is Form 5500?

Form 5500 serves as an annual return/report of an employee benefit plan. The form asks for basic plan information, including information about the plan sponsor and member counts. This form is required to be filed for employee benefit plans under sections 104 and 4065 of the Employee Retirement Income Security Act of 1974 (ERISA) and sections 6057(b) and 6058(a) of the Internal Revenue Code (the Code).

Who files Form 5500?

Our portal automatically generates a Form 5500 Data Summary Table for every group. This table compiles relevant information for the form, drawn from eligibility information we’ve collected throughout the plan year.

The employer (plan sponsor) is responsible for filing this form. The form is due by the last day of the seventh month after the plan year ends.

How can I find the Form 5500 Data Summary Table?

Employers should enter the Compliance tab on the portal to view the Form 5500 Data Summary. The employer is responsible for entering this information into the respective fields of the form before filing.
