What is Form MA 1099-HC?

Massachusetts has a state-specific version of a 1095 form called a 1099-HC. When filing state taxes, Massachusetts residents use this form to confirm that they had health insurance coverage during the previous calendar year. The form is due on January 31st of the following calendar year.

Who files Form MA 1099-HC?

Our portal automatically creates form MA 1099-HC for every Massachusetts resident who was a subscriber on one of our plans during any part of the previous calendar year. However, the subscriber is responsible for filing this form.

Any dependents filing individual income taxes may require information provided on the subscriber’s form. These forms may be photocopied.

How can I download Form MA 1099-HC?

Employers should enter the Compliance tab on the portal to obtain all forms MA 1099-HC for their subscribers. The employer is responsible for distributing these forms to their subscribers.

*Note: If a group has no residents in Massachusetts, this form will not appear in their Compliance tab.
