W-9 & 1099 Reporting 

As part of vendor onboarding, Yuzu requires a completed IRS Form W-9 from every organization we pay via ACH. This is a standard tax compliance requirement — we use the information on your W-9 to verify your business identity and file accurate 1099s with the IRS at year-end. 

What a W-9 Captures

A W-9 is a single-page IRS form that provides us with four key pieces of information:

  • Legal business name (as registered with the IRS)

  • Employer Identification Number (EIN)

  • Entity type (sole proprietor, LLC, S-corp, C-corp, partnership, etc.)

  • Signed certification that the information provided is correct

You are not required to apply for a new EIN to complete a W-9; if you already have one, simply enter it on the form. A blank W-9 can be downloaded directly from the IRS website: irs.gov/forms-pubs/about-form-w-9

Why We Collect It

As a Third Party Administrator making medical and health care payments, Yuzu is required by the IRS to file Form 1099-MISC for vendors we pay above the federal reporting threshold. The W-9 ensures we have the correct legal name, EIN, and entity type to file these forms accurately.

Without a W-9 on file, the IRS may require us to withhold 24% of your payments as backup withholding.

1099 Reporting

At the end of each calendar year, Yuzu files Form 1099-MISC with the IRS for vendors whose total payments meet or exceed the federal reporting threshold. You will receive a copy of any 1099 filed on your behalf by January 31 of the following year.

The information on your W-9 is used solely for tax reporting purposes and is stored securely.
