Employer Identification Number (EIN)

An EIN is required for every vendor or partner of Yuzu. We use it to verify your business and set up a secure account for your health plan funds. This is a tax and compliance requirement. Without a valid EIN on file, we cannot process payments to your organization.

An EIN is a 9-digit number assigned by the IRS to identify your business; it is not the same as a Social Security Number (SSN). If you are a sole proprietor and have been using your SSN for tax purposes, you may need to apply for an EIN through the IRS before completing this step.

W-9 Submission

In addition to entering your EIN in the portal, Yuzu requires a completed IRS Form W-9 from all vendors we pay via ACH. For details on what the W-9 captures and why we collect it, see our guide on W-9 & 1099 Reporting.

How to enter your EIN

  1. Select your profile (top left) and click Settings

  2. Under your organization name, select Banking

  1. Scroll to the EIN field and enter your number

  1. Save your changes
